when should a zerg player bring in swarm hosts if they do at all? are they worth it?

Swarm hosts should be brought out in mid-late game vs a ground ptotoss army and mech. You want to also combo thats with muta v mech and corrupters v toss.

To make your Swarm Hosts effective make sure you focus on creep spread before making them, and be ready do defend them from air. Generally you can do that by over making drones and going bananas with static defense on your massive creep spread. I support them with Queen/Roach/Hydra, plenty of static defense for my bases (since SH sucks at that) and a small pack of mutas (8ish) to harass and draw them out of position.

I find Swarm Hosts very hard to play, and mostly make them to throw my opponent off (its amazing how much a protoss will mess up his build once about 4 locusts start killing probes at his 3rd)

I would say midgame is a good time and yes, against ground compositions that you really cant fight other ways. I have found this DT > Immortal > Immortal+Archon build pretty much requires swarm hosts.
